CLINICAL EXPERIENCE OF MUSCULOCUTANEOUS ISLAND FLAP

Abstract
Compound muscle-skin flaps should, by derivation, be termed "Musculocutaneous" flaps.

The decision to use a musculocutaneous flap must be based on an accurate assessment of he defect and of the morbidity of the potential donor site. Basic principle of reconstructive surgery dictate that the plastic surgeon choose the optimal method of treatment for each particular patient. This involve a step-by -step consideration of all possible modalities of repair, from the simplest to the most complex. But these flaps are no different from all other flaps in that one should use the simplest method to achieve the desired result.

A thorough, detailed knowledge of anatomy is indispensable to the design of these flap. The recent realization that most skin blood supply is derived from perforating vessels from underlying muscles represents a radical departure from our previous concepts of flap physiology and design. Compare to random flaps, axial flaps have distintive advantage: they have good blood supply, wide arc of rotation and no necessity of delay. Recently anatomy of cutaneous axial flaps are now well documented which enables us popular use of musculocutaneous flap quate wound coverage becaue of its well cushion effect and good blood supply.

Here we present 8 cases of pressure sore which were treated with musculocutaneous flap. The results were satisfactory and acceptable.
